Any claim other than for insurance benefits under existing policies previously issued by Live Oak should be made to Andrew Hankins at 4321 Lake Street, Lake Charles, Louisiana 70605, 1-337-478-8687 must be made before October 1, 2026. The claim must include the name, address and phone number of the claimant, the basis of the claim and the date the claim arose.
